- Communicable diseases – Notifying individuals who may have been exposed to a disease or condition.
- Government functions – Cooperating with authorized federal officials for specific government purposes, such as military or veterans’ activities, correctional institutions, or workers’ compensation programs.
Uses and Disclosures With Your Authorization
Any other use or sharing of your personal health information will only happen with your written permission. You may change your mind and withdraw that permission at any time by submitting your request in writing.
Our Commitment
While we are not a HIPAA-covered entity, we voluntarily follow HIPAA privacy practices to protect your health information. This notice does not create legal or contractual rights.
Additional Notices
We may share your information with designated providers, including via electronic disclosure.
We reserve the right to de-identify your information and use it for any purpose.
We comply voluntarily with HIPAA best practices but are not legally bound as a Covered Entity or Business Associate.
